Purpose of the Position: A Case Manager is responsible for delivering the principles of the systems, protocols and the values of CBS to aid in the formulation of solutions, implementing guided strategies and maintain positive outcomes for referred behavioral cases while conducting the CBS program. 

Specific Tasks


1. Communication

- Conduct status check with parents/guardian before clinical sessions commence.

- Update parents regarding the children’s progress and concerns through verbal, written and SMS feedback (as requested).

- Communicate to teachers, therapists, tutors and other professionals with regards to behavioral concerns and goal setting.

- Attend case studies called for by the Programs Manager or Behavior Management Consultant.

- Constantly collaborate with the Programs Manager for program needs and concerns.

- Coordinate with the Administrative Officer for administrative needs and concerns.

- Coordinate with the collaborating schools’ regular teacher and/or special educators and therapists.

- Communicate with parents and provide updates about the collaborating schools’ requirements, schedules and policies.

- Manage communication with the child’s caregivers and other care providers under the care of the office.

- Report urgent matters to concerned parties.

- Coordinate with CBS Office for daily feed backing.


2. Inventory and Custody

- Maintain the safety conditions of the Center.

- Keep an inventory of toys, learning aids and intervention materials.

- Ensure the confidentiality of cases’ documents by safekeeping behavioral charts and files.


3. Documentation

- Accomplish behavioral charts and forms.

- Accomplish and submit reports such as Feedback Reports, Progress Reports and Incident Reports.

- Gather children’s output for their charts.

4. Behavior Management and Implementation

General

- Prepare communication materials.

- Facilitate proper pick up and drop off of the child.

- Ensure the child’s safety by conducting status check prior to one’s assignment. Check the child for marks, scratches and bruises and note any marks in one’s report. Check the child’s physiological status as well to determine readiness for intervention and notify parents and care providers if not.

- Usher the child properly during drop off time by taking the lead. Attend to the child until proper turnover is made to the parent, caregiver or teacher.


Clinical Intervention

- Provide learning accommodations and environmental modifications.

- Facilitate the child’s daily progress.

- Supervise home implementation for parents every last session of the week.

- Promote self-regulated behaviors.

- Implement the clinical protocols.

- Prepare daily activities for the children.

- Direct the shadow teacher to facilitate the goals.

- Provide mentoring and guidance of caregivers.

- Follow the Five Phases of Clinical Protocols

Shadow Teaching (One-on-One) 

Shadow Teaching (Mainstream) 

- Provide learning accommodations such as visuals, social stories, contracts, modes of communication and reinforcements.

- Provide academic assistance per need and per recommendation.

- Facilitate the child’s daily function in school as an integrated student in a mainstream setting.

- Supervise classroom attention and manage behavioral issues.

- Provide shadow assistance to encourage independence.

- Promote self-regulated behaviors and social behaviors.

- Facilitate classroom rules and norms, both existing and modified.

- Accompany the child in all school-related activities.

- Monitor the child’s academic competencies and needs.

- Provide assistive assistance and care.

- Conducts research and gathers references for the case.


Social Skills Facilitation

- Promote self-regulated and co-regulated behavior.

- Prepare activities for the group and manage the group dynamics.


5. Operational and Administrative Compliance

- Coordinate with the Administrative Officer with regards to administrative needs such as endorsement needs, correspondence materials, accounts facilitation and the like.

- Comply with schedule changes and assignments.

- Request budget for travel allowances.

- Log concerns in the Concerns Log Book.

- Ensure personal attendance.


6. Training – Human Resource Compliance

- Wear the prescribed uniform and ID.

- Ensure proper grooming and hygiene.

- Comply with the Professional Code of Conduct.

- Comply with the agreements made in the Training Agreement and Contracts.

- Attend all seminars, workshops and lectures called for by the Training Officer.

- Conduct research and reading to expand one’s concept and knowledge.
